Telemedicine Consent
I understand that telemedicine is the use of electronic information and communication technologies by a healthcare provider to deliver services to a patient whose location is different from that of the provider. An example of telemedicine is videoconferencing between patient and his/her healthcare provider.
Expected Benefits of Telemedicine:
Improved access to medical care by enabling a patient to remain at or closer to home instead of having to travel to the location of the healthcare provider.
More efficient medical evaluation and management.
Access to care or specialist(s) that are not otherwise available in a patient’s location.
Possible Risks of Telemedicine: As with any medical care option there are potential risks associated with the use of telemedicine. These risks include, but are not limited to, the following:
Delays in medical evaluation and treatment could occur due to deficiency or failure of equipment used to facilitate telemedicine; service interruptions; unauthorized access or other technical difficulties.
The images or audio transmitted may not be of sufficient quality to allow for appropriate medical decision making.
In rare cases, security protocols could fail, causing a breach of privacy of personal medical information.
Patient Consent to the Use of Telemedicine:
I understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine.
I understand that telemedicine may involve electronic communication of my personal medical information to other medical providers who may be located in other areas, including out of state.
I understand that I have the right to withhold or withdraw my consent to the use of telemedicine in the course of my care at any time, without affecting my right to future care or treatment. I understand that as long as this consent is in force (has not been revoked) Your Infinity Health may provide healthcare services to me via telemedicine without the need to sign another consent form.
I have read and understand the information provided above regarding telemedicine; have discussed it with my medical provider or such assistants as may be designated; and all of my questions have been answered to my satisfaction. I hereby give my informed consent to participate in telemedicine visit(s) and receive care via telemedicine from Your Infinity Health and its designated providers and staff.
SMS Text Privacy Policy
Protecting your privacy is important for Your Infinity Health. We realize that health is a personal, private subject matter, and we want you to be comfortable when communicating with us using SMS services (the “Services”). This SMS privacy policy (the “Policy”) applies solely to information collected by and through the use of the Services.
If you decide to opt-in to use the Services, you agree to abide by the terms and conditions of this Policy. The “Patient Text Message Consent Form” available here, is incorporated into this Policy. The Services are not meant to be used by children. By using the Services you are representing to Your Infinity Health that you are at least 18 years of age.
Your Infinity Health, at its sole discretion, reserves the right to make changes to this Policy without prior notice to you. The Policy, as updated, shall become effective as soon as posted on this website. Your continuous use of the Services after an amended Policy has been posted shall confirm your acceptance of all changes.
What personally identifiable information is collected from you through the Services, how is it used and with whom may it be shared?
YIH will only receive and collect information from you if you opt-in to use the Services, by completing the Patient Text Message Consent Form. YIH, when exchanging communicating with you through the Services, will receive and collect from you includes:
a. your mobile number;
b. your name;
c. your screen name, if any;
d. device identifiers; and
e. the text of the messages exchanged with users or sent to YIH.
The information received and collected by YIH, resulting from your use of the Services, will be stored and retained in our servers, which may be owned or serviced by third parties, consistent with YIH’s record retention policies, which may be periodically amended at YIH’s sole discretion, without notice to you.
How do we use your information?
YIH uses the information collected through the use of the Services to improve its services and enhance your health care experience. YIH may send you appointment reminders, service announcements, health education and awareness tips, for research activities and for anonymous reporting to our internal and external clients and partners, as allowed by law.
When can we disclose your information?
YIH will NOT share information you exchange with us via SMS (including personal identifiable information – name, phone number, etc.) with third parties, unless we have your written consent.
Every disclosure of your information is subject to federal and state legal requirements and shall conform to YIH’s Notice of Privacy Practice.
How secure is your information?
YIH strives to ensure the security of your information. However, not withstanding our technical and administrative safeguards, we cannot guarantee that malicious or unauthorized actors will not gain access to your data through illegal means or by accessing other platforms not within our control, that are used for the transmit information between you and YIH.
